Everything You’ll Need to Know
CCRES is pleased to offer a full range of benefits through the following carriers, effective January 1, 2026. Once you make your Open Enrollment elections, you cannot change your benefits until the next open enrollment period unless you experience a qualifying life event. Qualifying Life Events include marriage, divorce, birth/adoption of a child, death of a dependent, and/or change in employment status for you and a spouse. You must contact Human Resources within 31 days of the Qualifying Life Event if you wish to change your elections.
All employees (scheduled to work an average of 30 hours per week for medical and 20 hours for all other benefits) are eligible for benefits on the first of the month following 30 days of hire and will terminate on the last day of the month in which: your employment ends; your regular scheduled work hours are reduced to less than 30 hours per week; you stop paying your share of coverage.
To help make your care more affordable, benefit amounts under the indemnity plan (reimbursements paid to members) have been increased for hospital admissions, outpatient major surgery, anesthesia, office visits, and diagnostic lab services. We’ve also lowered the copays for emergency room and urgent care visits under the Super MEC plans.

Pram Rx. – Preventive Mandated Prescriptions: The “pram rx aca preventive drug list” refers to preventive medications covered under the Affordable Care Act (ACA) from a specific insurance plan provider, PRAM Insurance Services, Inc.. These lists are not universal; they vary by plan. Common categories of ACA-covered preventive drugs include contraceptives, folic acid and other prenatal vitamins, fluoride, low-dose aspirin, statins, smoking cessation aids, and certain vaccines.
Categories and examples of preventive drugs:
Contraceptives: Oral contraceptives (e.g., certain generic and brand-name pills), injectables, and emergency contraception options like levonorgestrel 1.5mg.
Fluoride: Supplements like sodium fluoride for children aged 6 months to 16 years.
Folic Acid: An essential prenatal vitamin.
Low-Dose Aspirin: For the prevention of heart attack and stroke.
Statins: Medications like atorvastatin for cholesterol prevention.
Smoking Cessation: Medications such as bupropion and nicotine replacement products like patches, gum, or inhalers.
Vaccines: Various vaccines like Pneumovax 23, Prevnar 13, Shingrix, and flu shots.
HIV Prevention: Medications for pre-exposure prophylaxis (PrEP).
Other preventive medications: Some plans may also include drugs for conditions like diabetes or high blood pressure, but coverage varies by plan.
How to find your specific list?
Click to check your insurance provider’s website: Search for your specific plan’s “preventive drug list,” “ACA preventive care drug list,” or “formulary”.
Contact PRAM directly: If you are a member of PRAM, you may need to contact them for a definitive list specific to your plan.
Review your Summary of Benefits and Coverage (SBC): This document provides an overview of your plan’s coverage, including preventive services.

Take the following actions to register with Lyric Health and add your dependent(s) to your coverage.
Adding dependents to your LYRIC HEALTH PLAN:
• Visit portal.getlyric.com or call 866-223-8831.
• Have the primary member’s date of birth, first name, and last name available.
• Follow the prompt to add dependent information. All dependents over the age of 18 will be emailed their personal account credentials to register once added under the primary member’s account.
