We encourage you and your family to become familiar with this website. Here you will find your insurance benefits, claim forms, links to the providers, and a link to the enrollment platform

User ID: Your Social Security Number 
Password (PIN): The last 4 digits of your Social Security Number followed by the last 2 digits of your birth year.


➢After reviewing the Benefits Confirmation sheet, please email Kara Davidson in HR at [email protected] with any changes,

questions, or discrepancies you see.  


➢The self-service is read-only. This site is available 24/7. You can’t modify your current elections, only during Active enrollment. Only update beneficiaries, and download your enrollment confirmation statement.


➢ You can request the Carriers’ contact information from HR and contact them directly or Click here to open the Insurer’s directory, if you have additional questions about ID Cards or if the member ID # is needed to start making doctor appointments.

Symetra Evidence of Insurability Online Form
Visit the online portal if an EOI is required for the Symetra Term Life coverage. If the EOI form is not submitted after completing your 2024-25 enrollment process, you will only receive the Guaranteed Issued amount.
