User ID: Your Social Security Number

Password (PIN): The last 4 digits of your Social Security Number followed by the last 2 digits of your birth year.

Please note: The self-service is read-only. This site is available 24/7. You can’t modify your current elections, only during Active enrollment. Only update beneficiaries, and download your enrollment confirmation statement.

2. Visit ccres.mybenefitsinfo.com/ to review the Benefit Overview prior to your scheduled appointment.

3. SSN Required for Dependents (spouse or children).

Symetra Evidence of Insurability Online Form

Visit the online portal if an EOI is required for the Symetra Term Life coverage. If the EOI form is not submitted after completing your 2024-25 enrollment process, you will only receive the Guaranteed Issued amount.
