As an active employee working at least 20 hours per week on average, you are eligible for Basic Life and AD&D Insurance. You MUST designate your beneficiary for the next plan year! – Life Insurance Benefit: $10,000 / AD&D Benefit: $10,000

Symetra Evidence of Insurability Online Form


Visit the online portal if an EOI is required for the Symetra Term Life coverage. If the EOI form is not submitted after completing your enrollment process, you will only receive the Guaranteed Issued amount.
