User ID: Your Social Security Number
Password (PIN): Last 4 digits of your Social Security Number followed by the last 2 digits of your birth year.
Symetra Evidence of Insurability Online Form
Visit the online portal if an EOI is required for the Symetra Term Life coverage. If the EOI form is not submitted after your enrollment process is completed you will only receive the Guaranteed Issued amount.
